Eligibility:
In order to use the services of the Chippewa Valley Free Clinic, several criteria must be met:
No reasonable health insurance or access to an alternative is possible
the scope of health care, such as VA, Medicare A or B benefits, or
disability.
Your household's income should not exceed 200% of
federal poverty level (FPL).
For example, if you have two people in
your family or your family, your income must be $31,860 or less.

the table below.
To confirm your eligibility, you must provide
proof of income.
This proof will include a tax return, pay slip, Social
Security declaration or declaration of unemployment.
Exceptions will be evaluated on a case-by-case basis.
Because
the Clinic strives to provide comprehensive and high quality care
for all patients, your first visit to the clinic will take about two
hours for most services.
The following steps are included
To determine your eligibility for clinic services, your financial situation
status will be reviewed by a Patient Attorney.
The Patient Advocate will
discuss with you the proof of income that you have or will have
to bring in.
Any other questions that you have can be addressed in this section.
After the meeting with the patient advocate, you will meet with
a registered nurse.
The nurse will conduct an initial examination (e.g. Reasons
on your visit to the hospital, blood pressure, etc.)
You will then find yourself with a
doctor or nurse who will perform a more thorough check-up and
make decisions with you about your health problem.
You can be
referred to other medical specialties (e.g., Eau Claire City-County
Health Department for family planning and STD testing/treatment) or to
resources in the community (e.g., food pantries, housing options, employment
opportunity).
The case manager or nurse will discuss these referrals
with you.
A Spanish interpreter is available.
You
will be treated with respect.
You are expected to respect all Clinic
staff and volunteers.
Verbal insults or combative behavior will not be tolerated
tolerated and will result in the immediate dismissal of the Clinic.
How do I make an appointment?
Patients who have been previously registered at the clinic can call
(715-839-8477) or stop during clinic hours to program an
appointment.
The clinic is open for your telephone call to make an appointment
Monday Thursday, 8:30 AM to 12:00 PM
Patients who have
not been previously registered at the Clinic are taken on a walk-in
every Tuesday night from 4:00 PM to 6:00 PM and 1st and 3rd Thursday
from 9:00 AM - 10:00 AM
New patients can only make appointments if
are registered patients.
They encourage new patients to arrive at the
start of registration because the clinic can fill up quickly.
