Hours: Mon, Tues, Thu 9:00 a.m. to 1:00 p.m. Wed and Fri 9:00 a.m. to 12:00 p.m. *Medical and Dental Clinic hours are subject to change depending on the availability of providers and holidays. Call and leave a number with the answering service and your call will be returned Intake Process: Medical and Dental services are by appointment only Must submit a completed qualification form before medical or dental treatment is offered Photo ID Proof of income with application Verification if receiving SNAP benefits, Unemployment, one month pay-stub, child support, alimony, SS (retirement or disability) Veteran disability or TANF Program Fees: No cost Our Dental Clinic provides free dental services including emergency extractions, fillings, and cleanings for the uninsured and those with low to no income. Our staff of dental assistants and hygienists work with a strong team of volunteer dentists from the community. Dental clinics are held throughout the week depending on the availability of our providers. Patients must submit an eligibility form that can be found on this website or picked up in front of the Medical Clinic Monday through Friday from 9 am to 1 pm. Once eligibility is determined, you will be scheduled for care.
