To access care, please contact a participating clinic in your area from the list below and request information on patient eligibility and available services.    How do I schedule an appointment? In order to schedule a new patient appointment, patients must call 706-425-4044 on Tuesdays from 4:00 pm - 5:00 pm. Appointments are scheduled every Tuesday based on availability. Patients are seen by appointment only and no walk-ins are accepted. Any person interested in scheduling a dental appointment must first become a medical patient by calling the appointment line on Tuesdays. Once you schedule your first medical visit you will be given information on how to schedule a dental appointment. Their dental services are limited to extractions, cleanings and fillings. They do not provide emergency dental care. Am I eligible? In order to be eligible for services, patients must be completely uninsured, at or below 150% of the Federal poverty level, and a resident in one of the following counties: Clarke, Barrow, Jackson, Madison, Oconee or Oglethorpe. What documentation is required?     Picture Identification    Proof of income, such as paystubs or a notarized letter    Proof of residency, such as a letter with a recent date, name and address, or a lease agreementAre there any fees? Services are provided free of charge, although every patient is given the opportunity to contribute to their care either through financial donations or volunteering. I am a current patient.
