Grand Peaks Medical and Dental is a non-profit Community Health Center dedicated to providing quality primary and preventative healthcare as well as health related services to all members of the community. Since Grand Peaks is a non-profit Community Health Center, they receive some funding to help support their services and allow them to offer discounts. All patients wishing to receive a discount must complete the Sliding Scale application and provide proof of household income. Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical center in 2007, with one Provider and a small staff. The Dental center was added in 2010. Have now grown to serve patients in St. Anthony, Rexburg and surrounding areas of the Upper Valley.
