Innis Community Health Center Inc. offers a sliding fee discount to all uninsured and underinsured individuals who qualify based on an approved sliding fee scale. This sliding fee scale provides a variable cost on certain service to individual that qualify. In order to apply for a sliding fee discount, the patient must provide household size and proof of total household income. Gross annual income is all income from all sources before taxes. This is required at the time of the visit to qualify for sliding fee scale. The Statement of Income declaration is required to qualify. Once eligibility is established, the patients charges will be discounted down to a minimum fee. All minimum fees are due at the time of service. Proof of income must be updated annually. All patients must submit proof of income each year in order to continue receiving discounted services.
