Provide proof of income. 3. Mail the application. Once your completed application is submitted with all the required documents, it will be reviewed and the information verified. When your application is approved, you will be called and have an appointment for a "new patient examination."     I am his mother and he doesn't live with me but I don't think he doesn't have a tooth that doesn't hurt. He doesn't work due to having migraines and bipolar. He has been diagnosed with bipolar but can't find a Dr or dentist to help him.

    Hello, I just moved here from Colorado Springs Colorado, I have Colorado medicad and obviously my insurance doesn’t work here and I do no quilfy for tenn care or medical here in Tennessee. I need help in a bad way I have a severely infected back tooth , my gum and jaw are swallowed , it keeps me up at night hurting. I can work for your services but can’t pay a lot .
