Dental Services:  Pediatric Dentistry Exams Cleanings Sealants Fillings Root Canals Extractions Crowns Bridges Dentures and PartialsTo qualify for the discount program, you must have a total family income that is less than 200% of the Federal Poverty Income Guidelines. To assist in assessing your eligibility for the program, you will need to provide the following required documents at your initial member registration: Proof of identification Income Residency They use an income sliding fee scale set by the Health and Human Services Commission to determine the percentage amount to be discounted for services.     Avoid this monster. No compassion. Laughs if you cry out in pain. Had to sell my wedding band to come up with the 65$ supposed sliding scale fee just for an exam. Wish i knew about J&A then. First exam is only 1$. After my rent and bills i have 24$ for the month. Thats for gas and food . I brought proof of that and they still demanded 65$ upfront just to look at my teeth. Sliding fee my butt.
